The Case Manager will manage the care of high risk members in the greater Nashville, TN and surrounding areas telephonically as well as in their homes or other community settings by completing health assessments, post hospital assessments, and coordinating patient care. The Case Manager will provide clinical and medical management services, including case management, care coordination, health assessments, interventions, and discharge planning. Work is generally fast paced, self-directed and not prescribed; this person will work from home when not completing face-to-face patient assessments.

If you are located in the Middle, TN area, you will have the flexibility to telecommute* as you take on some tough challenges. This position is both a Field Based and Telephonic Case Management position (primarily telephonic) which may require travel throughout the greater Davidson County, Nashville, TN area.

 
Primary Responsibilities:
  • Engaging members telephonically and in person (in their homes or facilities) to coordinate services, community resources, and treatment needs
  • Functioning as an advocate for members and decreasing unnecessary hospitalizations and emergency room visits
  • Travel in assigned territory as needed to complete patient assessments
  • Delivering a holistic approach to coordinated care based on the enrollee needs
  • Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file
  • Creating, reviewing and revising care plans and focusing on disease management
  • Collaborating with the member's PCP to deliver and coordinate necessary services
  • Building relationships with members and their families; assisting them with proper health choices
  • Ensuring cases are documented in a timely manner 

Required Qualifications:

  • Current, unrestricted RN license in the state of Tennessee
  • 1+ years of home care/home health and/or other case management experience
  • 3+ years of clinical experience
  • Computer / typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
  • Must be able to complete CCM certification within 30 months of your hire date
Preferred Qualifications:
  • Certification in Case Management (CCM)
  • Bachelor's Degree or greater in Nursing
  • Medicaid experience
  • Medicare experience
  • Experience managing chronic and/or complex diseases
  • Experience working in a health plan or other managed care setting
  • Experience with community health and / or public health
  • Long term care experience
  • Experience working with electronic medical records (EMR) systems
  • Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
  • Ability to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others
