The Field Based Community Health Worker is responsible for assessment, planning and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. They also Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services, and manage the care plan throughout the continuum of care as a single point of contact for the member. As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. The coordinator also addresses social determinant of health such as transportation, housing, and food access.
Working Schedule: Schedule is Monday through Friday any 8-hour 8:00 am to 5:00 pm.
This position is a field-based position with a home-based office. You will work from home when not in the field.
Location: Reside in the state of Virginia. Local travel up to 25% and mileage is reimbursed at current government rate.
If you are located in the state of Virginia, you will have the flexibility work in this hybrid role as you take on some tough challenges managing our pediatric population who require early intervention support services. This position acts as a liaison between the local lead education agencies and service providers
Primary Responsibilities:
- Engage members either face to face or over the phone to assist with closing gaps in care, linking to necessary services and providing early intervention support services
- Review available member service records and relevant documentation (e.g., utilization history, functional level, stratification information, current plan of care)
- Conducts pediatric health assessment
- Reviews IFSP (individual family service plans) and works to transition child at age 2.9 years
- Utilize interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessment
- Identify member service needs related to health concerns
- Identify urgent member situations and escalate to next level when necessary
- Engage family members to participate in the assessment process and collaboratively review and re-enforce with the family the IFSP plan
- Work with members to develop healthcare goals and identify potential barriers to achieving healthcare goals
- Review plan benefits and identify appropriate programs and services based on heath needs and benefits
- Work collaboratively with the interdisciplinary care team to ensure an integrated team approach
- Identify and refer members to relevant community resources
- Facilitate member choice of preferred provider
- Advocate for individuals and communities within the health and social service systems
- Performs other duties as assigned

Required Qualifications:
- Bachelor’s Degree (or higher) in a health-related field, Social Work, or Health Care Administration OR 1+ years of experience as an LPN
- 1+ years of experience with knowledge of the resources available, culture, and values in the community
- 1+ years of community outreach/field work experience in a health care setting
- Intermediate level of computer proficiency including the use of Microsoft Outlook, Teams, and multiple web applications
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information
- Valid driver’s license in Virginia and current automobile insurance with access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
- Ability to travel locally, up to 100 miles round trip and up to 25% of the time the state of Virginia
- Reside in the state of Virginia
- Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
Preferred Qualifications:
- Community Health Worker (CHW) Accreditation
- Experience working in Managed Care
- Knowledge of Medicaid/Medicare population
