The Quality Improvement (QI) Care Coordinator is responsible to the Quality Improvement (QI) Supervisor/ Manager and assists the company with improving quality standards. Support includes but is not limited to  telephonic scheduling of screening tests, educating patients about treatments, referral functions, reviewing electronic medical records, data entry, handling correspondence, and providing clerical support to the nurses in the department.  Corporate office setting for outbound calls and local travel is required. 

 

Primary Responsibilities: 

  • Consistently exhibits behavior and communication skills that demonstrate commitment to superior customer service, including quality and care and concern with each and every internal and external customer
  • Represents the Company in a professional manner, following all Company policies and procedures
  • Uses, protects, and discloses patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
  • Calls patients to assist in scheduling appointments and/or conducting assessments related to their medical care
  • Conducts Retinal Camera imaging on site at clinics with patients directly
  • Communicates with patient regarding medical screenings and able to educate both patient and family regarding rationale and importance of screenings as directed by the physician
  • Completes all administrative functions associated with referral activities in a timely manner
  • Maintains patient medical records in accordance with Company policy and maintains proper assembly of medical records
  • Follows Company’s standing orders and protocols for proper authorization and processing of referrals
  • Assists patients with external resources when needed
  • Communicates with patient on a timely basis to meet quality measures and scheduling requirements
  • Communicates professionally with medical offices. Receives consultant reports, maintains documentation, and routes to the appropriate physician promptly
  • Monitoring referral reports not received and timely follow-up in accordance with Company policy and procedure
  • Using various data sources enters data into spreadsheets or programs
  • Maintains call logs, excel spreadsheets, and data findings electronically
  • Compiles, organizes, and mails out materials to internal and external customers 
  • Performs general administrative duties such as typing correspondence, creating or updating spreadsheets, photocopying, faxing, and compiling data and other materials or other duties as required
  • Must have consistent, punctual and reliable attendance
  • Ability to travel up to 3 days per week locally to centers
  • Performs other duties as assigned

Required Qualifications: 

  • High School diploma, GED or equivalent
  • Demonstrated intermediate proficiency in Microsoft Office (Word, Outlook)
  • Demonstrated intermediate proficiency in Excel spreadsheets
  • Ability to travel 25-50%, as needed for business
  • Ability to work onsite in an office environment 
  • This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor’s diagnosis of disease 

Preferred Qualifications: 

  • Medical Assistant or Certified Nursing Assistant
  • Experience working as a Quality Improvement (QI) Care Coordinator
  • Medical office experience with referral processing for HMO plans
  • Proven knowledge of the EMR system
  • Proven knowledge of HIPAA regulations
  • Bilingual (English/Spanish)
  • Demonstrated excellent organizational skills with the ability to multi task, set priorities, and meet deadlines
  • Ability to be results and goal-oriented with a philosophy for quality improvement
  • Demonstrated exceptional telephone skills
  • Ability to communicate effectively with patients and all levels of personnel in a professional, courteous and effective manner using excellent customer service skills
  • Ability to follow written and oral instructions and to work with general guidance
  • Ability to read, speak, write, and understand the English language fluently
  • Ability to possess a high degree of accuracy, efficiency, and dependability  

The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
