Positions in this function include Hawaii state licensed RN, LSW, or other qualified licensed behavioral health providers.  Individuals in this role will liaison with the Department of Public Safety (DPS) and other community partners to assist members in successful transition from jail, prison, or the Hawaii State Hospital. This individual will be a SME to the health plan clinical teams and work collaboratively with the Health Coordination Managers, Health Coordinators, Housing Manager, and others to ensure successful transition of members into the community.

The Justice Liaison will be responsible for developing relationships with DPS and other social service providers to increase the supports available to our members.  The Justice Liaison will help identify health plan members that are eligible for the Community Transitions Program.

Primary Responsibilities:

  • Conduct comprehensive member assessments
  • Engage and assist members in the justice system transitioning into the community
  • Engage and assist members in accessing supports, services, and housing as they transition from the justice system into the community
  • Engage members either face to face or telephonically to have a discussion about their health
  • Collaborate with member to create solutions to overcome barriers to achieving health care goals
  • Provide individualized education on preventative healthcare measures
  • Review plan benefits and identify appropriate programs and services based on health needs and benefits
  • Engage member/providers to participate in the assessment process and collaboratively develop a person centered Health Action Plan based on their individual needs, preferences, and objectives
  • Coordinate member access to health-related programs, services, and/or providers
  • Document information in relevant systems
  • Coordinate with clinical staff to facilitate member care and/or services
  • Facilitate member choice
  • Determine and assist in maintaining member eligibility

Required Qualifications:

  • RN, LSW, or other licensed behavioral health provider with a current and unrestricted Hawaii licensure
  • 3+ years of experience working with the justice involved population
  • 2+ years of experience with Medicaid/Medicare populations, including LTC and TANF/CHIP
  • 2+ years of clinical or health coordination experience applicable to the Medicaid managed care population
  • 2+ years of experience in field-based case management
  • Experience working with diverse populations
  • Substantial understanding of community resources
  • Solid knowledge of local and community-based social service organization
  • Proficient in the use of software applications including, but not limited to MS Office Suite (Outlook, PowerPoint, Excel, Word, Project)
  • Access to reliable transportation
  • Proven ability to work independently, managing multiple and often competing demands
  • Proven ability to multitask in a fast-paced environment
  • Proven ability to work with multi-disciplinary clinical teams
  • Proven ability to collect and analyze data and write reports
  • Proven ability to present on a variety of topics to different stakeholder groups

Preferred Qualifications:

  • BSN, LCSW or other licensed behavioral health provider
  • Certified Substance Abuse Counselor (CSAC)
  • Experience with behavioral health, substance abuse, and dual diagnosis populations
  • Understanding of the justice system processes
  • Proven to developed relationships with the Department of Public Safety entities

The salary range for this role is $71,600 to $140,600 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
