Field Based Patient Care Coordinator – Nassau OR Suffolk County, NY

Primary Responsibilities:
- Support interdisciplinary care team with overall member coordination at the direction of RN or NP/PA
- Data entry and member chart management for: New members, post hospital visits, quality, labs and medications
- Obtain medical records following hospitalizations or physician appointments
- Scheduling member transportation or appointment

Required Qualifications:
- High school Diploma / GED or equivalent work experience
- Must be 18 years of age or older
- 3+ years of clinical or medical record experience. Strong knowledge of medical terminology and EHR platforms
- Access to reliable transportation that will enable you to travel to patient locations
- Reside within Nassau, Suffolk County
- Ability to travel up to 100% of the time

Dedicated work area established that is separated from other living areas that provides information privacy

Residing in a location that can receive a high-speed internet connection (additional information on UnitedHealth Group approved internet providers and restrictions will be provided)
- Ability to work full-time, Monday – Friday. Employees are required to have flexibility to work our normal business hours of 8:00am – 5:00pm, no nights, weekends, or holidays.
Preferred Qualifications:
- Certified nursing assistant or medical assistant certifications
The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
