Community Health Worker – Remote in Texas

This position is working with STAR / CHIP members with Special Healthcare Needs. In this Health and Social Services Coordinator role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the members’ needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.

Must be in Texas and be able to travel to the local office /community about 10% of the time based on business needs. 

If you reside within a Texas, you will have the flexibility to telecommute* as you take on some tough challenges. 

Primary Responsibilities: 

• Assess, plan, and implement care strategies that are individualized by patients and directed toward the most appropriate, lease restrictive level of care
• Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
• Manage the care plan throughout the continuum of care as a single point of contact
• Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
• Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team

Required Qualifications: 

• High School Diploma/GED (or higher)
• 3+ years of experience in a health care role
• Intermediate level of Microsoft Office proficiency including Microsoft Word, Excel and Outlook (ability to create, edit, save and send documents, spreadsheets and emails)
• Live in a location that can receive an approved high-speed internet connection or leverage an existing high-speed internet
• Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
• Ability to travel in assigned regions to visit Medicaid members in their homes, and/or other settings, including community centers, hospitals, nursing facilities or providers’ offices
• Ability to travel up to 10% of the time for team meetings, company events or member engagement

Preferred Qualifications:  

• Certified Community Health Worker (CHW)
• Experience with Texas STAR and CHIP
• Case management experience
• Call center experience
• Experience working with members with a serious mental illness (SMI) and/or previous experience with members with mental illness
• Experience in managed care
• Bilingual English / Spanish / Chinese/ Mandarin

The salary range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
