Primary Responsibilities: 

• Owns relationship & is the single point of contact for hospitals with on-site engagement
• On-site engagement defined as:
  • o    EMR access
  • o    Access to patients
  • o    Engagement with hospital CM, D/C Planers, Social Workers, & Hospitalists
  • o    Access to hospital census is preferred 
• Owns prospective SNF requests that originate from their designated hospital(s)
• Approves prospective request within established parameters
• ACC forwards request to UM Partner if outside auth parameters or they are unable to complete the request within 1 hour

Required Qualifications:         

• Registered Nurse or Registered Therapist (PT, OT, ST) with current licensure to practice in the applicable state
• 3+ years of clinical experience.
• Proficient with Microsoft Office applications including Word, Excel and PowerPoint.
• Proven independent problem identification/resolution and decision-making skills.
• Proven detail oriented
• Proven team player

 

Preferred Qualifications:     

• Case Management experience with CCM 
• Experience working with geriatric population
• Patient education background, rehabilitation, cardiac care, and/or home health nursing experience
• Proven exceptional interpersonal and communication skills.
• Proven solid problem solving, conflict resolution and negotiating skills.

 

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $xx.xx to $xx.xx per hour based on full-time employment. We comply with all minimum wage laws as applicable.
