The Member Clinical Support Coordinator – Transitions Administration plays a critical role in supporting key administrative functions across the Community Transitions Program (CTP), Optum Transitions to Home (OTTH), and the Medication Reconciliation Program (MRP). This position ensures smooth coordination between members and clinicians by managing essential tasks that facilitate timely access to transition of care services.

This role involves outbound calls, extracting and analyzing data from multiple Excel reports and processing it within Electronic Medical Records (EMR) systems, following specific guidelines tailored to each transitions program. Accuracy and attention to detail are vital to ensure timely notifications and assignments that support member care. Operating in a fast-paced, high-volume environment, this role emphasizes teamwork, quality, and performance. It is a virtual position, allowing you to work from the comfort of your home while contributing meaningfully to care coordination efforts.

This position is full-time (40 hours/week) Monday – Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 7:30 am – 4:00 pm. It may be necessary, given the business need, to work occasional overtime.

We offer 2 weeks of on-the-job training. The hours of training will be aligned with your schedule.

 

Primary Responsibilities:

  • Assess member eligibility daily to the appropriate transitions program, ensuring continuity of care in collaboration with transitions clinicians.
  • Make outbound calls to providers and pharmacies
  • Research internal and external resources-such as pharmacies and primary care provider offices-to validate contact information for dialer exclusion members.
  • Accurately document relevant information within Electronic Medical Records (EMRs), including systems such as ICUE and Pathway.
  • Efficiently navigate and operate multiple technology platforms to support workflow, including CommunityCare, Doc360, nH Coordinate, and Maestro.
  • Maintain confidentiality of health information in compliance with state and federal regulations, including HIPAA.
  • Adhere to established policies and procedures aligned with regulatory and organizational standards.
  • Uphold a strong commitment to quality in all aspects of work.
  • Demonstrate consistent schedule adherence and reliable attendance.
  • Meet or exceed performance metrics related to quality, productivity, and outcomes.

Required Qualifications:

  • High School Diploma / GED OR equivalent work experience
  • Must be 18 years of age OR older
  • 1+ years of experience in healthcare, social work, community outreach, or advocacy.
  • 1+ years of experience working with Medicare and/or Medicare/Medicaid populations, including DSNP and CSNP membership.
  • Proficiency in Microsoft Office applications:
    • Word – create, edit, and save documents
    • Excel – sort and filter data
    • Outlook – manage email, folders, attachments, and calendar functions
  • Ability to work five days a week, Monday through Friday, during our normal business hours of 7:30am – 4:00pm CST.

Telecommuting Requirements:

  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $20.38 – $36.44 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
