  • We provide or assess care in a variety of settings and situations including telephonically, in patients’ homes, on-site at skilled care facilities, long term facilities, medical practices, urgent care settings, and via telemedicine.
  • We place a premium on maximizing time with the patient to understand their needs, concerns and issues.
  • Many of our jobs require diplomacy as you coordinate care with primary care givers and support teams.
  • You will need to spend a considerable amount of time at the computer or tablet for planning, documentation, training and team coordination.
  • We take on ambitious goals and considerable workloads so you’ll need to have a strong sense of accountability and be very self-directed.
  • Some of the roles here involve complex life situations that are emotionally challenging.

What We Are Looking For

These jobs aren't easy; but if you're looking for a challenging and rewarding environment, we're looking for elite performers.

  • Nurse Practitioners must have a current, unrestricted license in the state of assignment or ability to obtain.
  • Physician Assistants must have a current, unrestricted (PA-C) license in state of assignment or ability to obtain.
  • We hire new  graduates as well as experienced  advanced practice  care providers.
  • Most of our positions require gerontology, adult, family or acute specialization. However, some other specialties may be necessary such as pediatric, OB/GYN, urology, etc.
  • National certification or ability to obtain is required.
