Position Highlights:

• Schedule: Part-time, approximately three days per week
• Meet with members to counsel and educate regarding health plan benefits, health plan network and other programs and tools available to help improve quality of lifestyle
• Conduct assessments, annual wellness exams, high risk exams and transition of care appointments with patients (in clinic, IPA office, SNF, home setting or virtual). Document chief complaint, previous medical, family, and social history, review of systems, examination, assessment, and appropriate plan of care.  Responsible for the coordination of care with specialists and appropriate ancillary services
• Complete all documentation and paperwork in a timely manner. Maintain quality of care standards as defined by the medical group
• Identifies members needs and any gaps of care; provide recommendation of benefits/ programs associated with member’s health plan and guide members appropriately

Required Qualifications: 

• Master’s degree from four-year college and/or a professional certification beyond a four-year college
• Certificate of completion accredited Nurse Practitioner Program
• Current ANCC and/or AANP Certification required
• Unrestricted CA PA or NP license
• Current CA DEA certificate required prior to start date
• Active BLS certification
• 1+ years experience in outpatient setting
• EMR Proficient
• Excellent patient care and time management skills
• Flexible, multi-prioritizing skills and time management skills
• Ability to work independently and without direct supervision

Preferred Qualifications:  

• Fluency in Spanish
• 1+ years experience; preferably in outpatient/managed care setting
• Working knowledge of managed care, health plans, medical groups, IPA networks 

California Residents Only: The salary range for this role is $109,513 to $160,000 per year.  Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
