The Supervisor, Social Determinants of Health is responsible for oversight, leadership, and development of screening methods and care delivery for social determinants of health (SDOH). They will ensure referrals and interventions successfully improve patient outcomes of our global risk patient population. This role will lead a team of social workers and care team associates to receive centralized referrals from the care management team and/or providers. As a leader, the supervisor will champion SDOH as a crucial part of healthcare for our transitions of care and longitudinal care management programs.

Primary Responsibilities: 

  • Create an “above and beyond” culture to ensure patients share their SDOH-related needs, gain access to resources, and can better manage their healthcare
  • Establish process for SDOH team to make closed-loop referrals to community resources, health plan benefits, other services, and longitudinal programs
  • Identifies opportunities for process improvement for referral intake, documentation, and care coordination
  • Increase referrals from clinicians and successful usage of transportation, food security, financial, housing, and other SDOH interventions
  • Identify opportunities to develop and pilot new SDOH initiatives with internal resources and vendors that impacts clinical outcomes
  • Coordinate with care management supervisors to provide seamless patient experience and execute team priorities
  • Educate care managers and providers to better identify SDOH needs and refer for assistance
  • Establish and build key relationships with community-based organizations to execute SDOH strategy
  • Participate in weekly discussions and monitoring of analytics regarding the program
  • Regularly present referral management, productivity, and operational data to departmental leadership
  • Provide oversite of the team including but not limited to schedules, coverage, and productivity
  • Support data collection and quality metrics as assigned and assist healthcare team in meeting quality metrics

Required Qualifications: 

  • 3+ years of experience with direct patient or community member interaction
  • 1+ years of supervisory experience
  • Ability to travel quarterly for group meetings
  • Reside in the Tri-state markets (NY/NJ/CT)

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
