Performs a variety of activities that support the clinical, business and program related work of the clinical staff in the Care Management Department. Collaborates with clinical and nonclinical staff across multiple sites within the organization to ensure timely contact of patients discharged from hospital and skilled nursing facilities to assess needs of patient and/or family and documents contact in appropriate database. Communicates outcomes to appropriate Care Managers or others.

 

This position is full time, Monday – Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00 am – 4:30pm or 9:00 am – 5:30 pm .It may be necessary, given the business need, to work occasional overtime. Employees will be required to work some days onsite and some days from home.

We offer weeks of on-the-job training. The hours of the training will be based on schedule or will be discussed on your first day of employment.

Primary Responsibilities:

  • Supports Care Management department functions; assists and cross-trains co-workers on entering referrals for Care Management.
  • Makes follow-up phone calls to discharged patients, post-acute admissions and communicates identified issues to Care Management/Social Workers or others for timely intervention/resolution following established standards.
  • Schedules Primary Care Physician and Specialist appointments when available for patients identified as needing follow-up appointments after discharge from an inpatient or skilled nursing facility.
  • Supports multiple office functions and provides cross-coverage for entering department referrals as required.
  • Based on daily Hospital and Skilled Nursing Facility Discharges, determines patient category utilizing pre-established guidelines and calls patient and/or family to assess needs within position responsibility.
  • Documents in computerized medical record input from a patient and family for MA10 Transition Coordinator 2 other health team members.
  • Assists health team members in the formulation of patient care plan within position responsibility.
  • Relays information, questions / concerns to and from patients, Care Managers and others.
  • Escalating issues to Director or others as required.
  • Documents communications and resolutions in appropriate database per department standards.
  • Represents the department as needed on interdepartmental meetings and committees when requested.
  • Maintains patient, staff and institutional confidentiality.
  • Complies with health and safety requirements and with regulatory agencies such as DPH, etc.
  • Complies with established departmental policies, procedures, and objectives.
  • Performs other similar and related duties as required or directed.
  • Regular, reliable and predicable attendance is required.

Required Qualification:

  • High School Diploma / GED
  • Must be 18 years of age OR older
  • 3 + years relevant experience
  • Certified Medical Assistant, EMT, OR licensed LPN in the State of Massachusetts
  • Basic clinical skills and demonstrated excellent customer service skills.
  • Proficient in various software applications such as Microsoft Office Excel, Microsoft Word and Microsoft Outlook.
  • Knowledge of  Microsoft Excel spreadsheets with ability to sort data.
  • Must consistently exercise sound decision-making skills.
  • Must have ability to multi-task, prioritizing daily workload and seek assistance when necessary.
  • Ability  to work any of our 8-hour shift schedules during our normal business hours of 8:00 am – 4:30 pm or  9:00 am – 5:30 pm . It may be necessary, given the business need, to work occasional overtime 

 

Preferred Qualifications.

  • Electronic Medical Record experience preferred.
  • Excellent organizational, interpersonal, written and verbal communication skills.
  • Effective problem-solving skills.
  • Demonstrated ability to learn new processes quickly.
  • Proven experience in telephone contact with patients within an ambulatory practice setting.

 

Telecommuting Requirements:

  • Reside within commutable distance to the office at 100 Front Street Worcester. MA
  • Ability to keep all company sensitive documents secure (if applicable)
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

The hourly range for this role is $16.88 to $33.22 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
