Position Highlights:
- Practice Primary Care to the full scope of your license serving 18+
- Full-time position; 36 patient contact hours weekly
- 18-22 patients per day on average
- EPIC EMR system with Dragon dictation
- Dedicated MA for rooming and support
- Onsite lab and pharmacy resource support for medication refills
- Clinic hours are Monday-Friday, 8am-5pm
- Answer urgent/emergent patient calls after hours by phone, one week every 2-3 months

Required Qualifications:
- Unrestricted Arizona NP or PA license or ability to obtain by start date
- Current DEA registration or ability to obtain by start date
- Current BLS certification
- National Nurse Practitioner certification through AANC or AANP, or Physician Assistant national certification through NCCPA
Preferred Qualifications:
- Experience in providing patient care under a Value Based Care Model
- Experience in Primary Care
- Knowledge of HEDIS measures, CAHPS, HOS, HCC Coding documentation
- Proficient in Medicare Annual Wellness visits
- Proficient in EMR systems & technology
The salary range for this role is $104,500 to $156,000 annually based on full-time employment. Salary Range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
