As a Nurse at Optum, you will be responsible for providing professional nursing care by assessing, planning, implementing, and evaluating the care of patients under the supervision of a clinician. You will delegate tasks as needed to professionals and para-professional teammates. As a nurse, you will coordinate activities and work closely with clinicians and teammates to maintain efficient department functions and ensure the successful operation of the department. 

Primary Responsibilities:

• Oversight of all palliative care vendors in the Inland Empire
• Monitor palliative care pursuit lists and ensuring referrals for appropriate members  
• Monitoring data surrounding palliative care metrics to align with market metrics
• Attend and manage IDT rounds with all palliative care vendors
• Bridge relationships between palliative care teams, local PCP’s, and specialists
• Lead staff to follow clinic policies and procedures and directs strategy and manage non-clinical staff

Required Qualifications: 

• Graduation from an accredited school of nursing
• Active, unrestricted Registered Nurse license through the State of California
• Palliative care and hospice experience

Preferred Qualifications: 

• BSN Degree
• 1+ years of experience working as a Registered Nurse

California Residents Only: The hourly range for this role is $28.03 to $54.95 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
