Case Manager II’s provide home-based services, education, and support to families, including household management and emotional/behavioral regulation. You will administer program assessments, assist in screening and orientation of new patients and families, and coordinate services within and outside the clinic. Case Manager II’s also define problems, collect data, and interpret technical instructions.

Primary Responsibilities:

  • Provide home-based services, education, and support to families
  • Travel independently for home visits, school, office visits, or other necessary facilities
  • Participate in monthly observations and weekly supervision with MHP Supervisor
  • Schedule psychiatric and PCP appointments to maintain compliance with standards
  • Coordinate services within and outside the clinic (counseling, medication management, welfare, vocational services
  • Implement and follow intervention plans written by a therapist
  • Submit documentation within 24 hours of service, follow mandatory reporting laws, and maintain weekly billing

Required Qualifications: 

  • Current CPR & First-Aid Certification or ability to complete BLS certification within 30 days of hire
  • 1+ years of experience in the Human Services field
  • Experience working with computers for professional communication and medical documentation – Excel, Outlook, Athena RMS (or other EHRs)
  • Access to reliable transportation, as well as a valid driver’s license, that will enable you to travel to client and/or patient sites within a designated area

Preferred Qualifications: 

  • Experience with clients that present with various issues; substance abuse, domestic violence, trauma recovery, anxiety, depression, relationship problems with family or significant others, stress management, homelessness, and/or lack of resources
  • Ability to define problems, collect data, establish facts, and draw valid conclusions
  • Ability to interpret technical instructions in mathematical or diagram form and deal with abstract and concrete variables
  • Excellent written and oral communication skills
