As a RN Field Care Coordinator, you will be an essential member of an Integrated Care Model team addressing both medical needs and social determinants of health by relaying the pertinent information about members’ needs, advocating for the best possible care available, and ensuring they have the right services to meet their needs.

If you are in the Suffolk/Norfolk, MA areas, you will have the flexibility to work a Hybrid schedule as this is a Field-based opportunity with a Home-based office as you take on some tough challenges.

 

THIS IS A FIELD-BASED ROLE WITH A HOME-BASED OFFICE

 

Primary Responsibilities:

  • Assess, plan, and implement care strategies for members that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
  • Manage the care plan throughout the continuum of care as a single point of contact
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team  
  • Expect to spend about 50-80% of your time in the field visiting our members in their homes or in long-term care facilities

Required Qualifications:

  • Current, unrestricted RN license in MA plus 2+ years of clinical work experience
  • 2+ years of experience in long-term care, home health, hospice, community health, or assisted living
  • 2+ years of experience working with MS Word (create and edit) and Outlook (email and calendar)
  • Ability to travel in assigned region to visit members in their homes and/or other settings, including community centers, hospitals, or providers’ offices
  • Access to high-speed internet at residence

 

Preferred Qualifications:

  • BSN degree or working toward a BSN
  • Experience with electronic charting
  • Field based work experience
  • Background in managing populations with complex medical or behavioral needs
  • Bilingual
