The RN Clinical Care Coordinator will perform activities that focus on Foster Care and Kinship Medicaid members, relaying the pertinent information about the child’s needs, advocating for the best possible care available, and ensuring they have the right services to meet their needs.

This is a home-based position with possible field responsibilities.

Primary Responsibilities:

  • Partner and collaborate with the internal care team, providers, and community resources/partners to implement care plan
  • Advocate for members and families as needed to ensure the member’s needs and choices are fully represented and supported by the health care team
  • Work independently to identify priorities, make decisions, and complete work to meet established objectives; Works with minimal guidance; seeks guidance on only the most complex tasks  
  • Maintains records identifying individual member services that were missed
  • Prepare reporting required by organization and state partner
  • Other responsibilities as needed

Required Qualifications:

  • Current, unrestricted independent licensure as a Registered Nurse in Ohio
  • 4+ years of Clinical Experience
  • 1+ years of experience working with children in custody, adoptive children and/or the pediatric population
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • Reside within Allen, Auglaize, Butler, Champaign, Clark, Clermont, Clinton, Darke, Defiance, Fayette, Fulton, Greene, Hamilton, Henry, Logan, Madison, Mercer, Miami, Montgomery, Paulding, Preble, Putnam, Shelby, Union, Van Wert, Warren, Williams Adams, Athens, Belmont, Brown, Delaware, Fairfield, Franklin, Gallia, Guernsey, Highland, Hocking, Jackson, Knox, Lawrence, Licking, Meigs, Monroe, Morgan, Muskingum, Noble, Perry, Pickaway, Pike, Ross, Scioto, Vinton, Washington of Ohio, or the very close surrounding areas, with reliable transportation and the ability to travel within the designated area if needed

Preferred Qualifications:

  • BSN, Master’s Degree or Higher in Clinical Field
  • Certified Case Manager (CCM) certification
  • 1+ years of case management experience coordinating care for individuals with complex needs 
  • Experience working with external organizations geared toward supporting children such as Protective Children’s Services agencies, Families & Children First Experience/exposure with discharge planning, utilization review, concurrent review and/or prior authorization review
  • Experience with arranging community resources
  • Background in Managed Care  
  • Councils, Boards of Developmental Disabilities, etc.

The hourly range for this role is $28.61 to $56.06 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
