Care Plus Program – Advanced Practice Clinician, Optum CA – San Fernando Valley

Position Highlights:
- Care Plus Acute
- Goal is to manage patient’s medical needs as they transition back from an inpatient stay to the community by having a face to face visit every week
- Program is an added layer of support along with the PCP and care is transitioned back to the PCP after 30 days
- Goal is to manage patient’s medical needs as they transition back from an inpatient stay to the community by having a face to face visit every week
- Care Plus Primary
- Homebound program where a patient from the acute program can also be transitioned to if they are still unable to seek care in the community after 30 days
- Patients who have not had a face-to-face visit with PCP or specialist in the last 6 months
- Patients are seen quarterly and PRN
- Homebound program where a patient from the acute program can also be transitioned to if they are still unable to seek care in the community after 30 days
- 6-7 average patients per day | 60 mins for new patients and 30 min follow up appointments
- Patient mix of recently discharged patients from inpatient setting and homebound patients
- No weekend or after-hours clinics; phone call required for treating established patients
- No on-call duty
- Majority of the time will be spent visiting patients in their home environment
- Review discharge summaries
- Review Past and Current Medical History
- Engage the patient and conduct appropriate clinical assessment
- Medication Reconciliation
- Identify specialist and other pertinent follow up
- Develop a plan of care
- Order necessary testing that patients may need
- Discuss and address potential acute issues

Required Qualifications:
- Certificate of completion from an accredited Nurse Practitioner or Physician Assistant Program
- Unrestricted California Advanced Practice Clinician license
- Current AANP/ANCC/NCCPA certification
- Current California DEA certificate required or ability to attain prior to start date
- 2+ years of experience in ambulatory care, ER, ICU
- EMR Proficient – Currently using Touchworks, but will be switching to EPIC in September 2025
- Excellent patient care and time management skills
- Ability to work in fast-paced environment, team player, adaptable to changing priorities
- Reliable transportation for daily travel (travel reimbursed at IRS rate)
Preferred Qualifications:
- Home Health Nurse experience
The salary range for this role is $104,500 to $156,000 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
