Field Based Community Health Worker – Houston, TX

This position is full time (40 hours/week) Monday – Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00 am – 5:00 pm. It may be necessary, given the business need, to work occasional overtime. Our office is located at HARRISSUITE 220, 4700 W SAM, Houston, TX
We offer weeks of paid training. The hours of the training will be based on schedule or will be discussed on your first day of employment.
Primary Responsibilities:
- CHW receives referral via eCW (tcon, AI, referral) for identified SDOH needs on enrolled CareCoach Connect Patients. All new admits to CareCoach Connect longitudional program will be evaluated telephonically for any social service needs and a a home visit will be at SW/provider discretion. The HV will be conducted by the SW or CHW. A referral does not need to be generated for new admits.
- CHW will complete the Initial Home Visit/On-site Assessment which includes but not limited to: Home Environment, Support System/Self Sufficiency, Advanced Care Planning, Caregiver SOS
- Follow-up: 1. Based on assessment findings, the CHW will develop interventions and actions related to the needs of the patient and family members. 2. Document whether follow up is needed, what type and at what frequency 3. Frequency of follow-up with the patient is based on patient’s need, request by CareCoach Connect Provider, RN, or SW’s/CHW’s discretion 4. APS involvement warrants follow-up. Follow-up frequency is based on severity of neglect/need for placement. Follow-up frequency determined at SW discretion. Follow-up is primarily with assigned APS worker. APS follow-up will be completed by assigned SW. 5. Routine referral for community resources follow-up within 10 business days if needed; based on Community Healthcare Worker discretion.
- Community Healthcare Worker will incorporate the appropriate resources that can help the patient, the following is a list of services, but not limited to: 1. Transportation assistance: Public transportation application, insurance benefit contact information, transportation resources in the community 2. Meal Supplements: Meals on Wheels, Food bank, etc. 3. Housing 4. Personal Care Attendant Services-Private pay or Medicaid paid 5. Medication assistance programs 6. Any additional programs/services that are available within the Market
- CHW to communicate findings in writing in eCW. All Clinical notes must be locked within 72 hours. Results of assessments may also be shared verbally or a written summary during weekly IDT.
- Community Healthcare Worker will document unsuccessful attempts to reach patient in Referral/ Telephone Encounter.
- Community Healthcare Worker will communicate with CCC SW as needed.

Required Qualifications:
- High School Diploma / GED
- Must be 18 years of age OR older
- CHW Certification. (Community Healthcare Worker)
- Texas Driver’s License
- Currently reside in commutable distance within Houston, TX
- Must have experience within the community area for at least 2 years

The hourly range for this role is $19.86 to $38.85 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
