Schedule: Monday to Friday, 8 AM-5 PM

Location: Onsite – 5714 Spohn Dr, Corpus Christi, TX 78414

The Patient Care Coordinator – Medical Records is responsible for providing general functions that include patient telephonic outreach and greeting with the purpose of scheduling visits, patient registration and consent, insurance verification, and scheduling of diagnostic and follow-up appointments to support the goals of the Practice Support Unit (PSU). A primary focus of this position is to support the medical records to ensure PCPs obtain PSU visit results within seven days. This position manages all record requests and PSU productivity reports.

Primary Responsibilities:

  • Patient telephonic outreach to schedule patients for PSU health events to provide annual screenings of quality measures
  • Collect and verify demographic information, insurance eligibility including plan co-insurance, language and ethnicity preference for future contact
  • Timely notification and reminder calls for upcoming appointments to include location of event and landmarks for easy identification of PSU facility
  • Call follow-up with patient will include rescheduling appointments that were cancelled, rescheduled and no-shows
  • Enter dispositions for 100% of calls made to patients in WellMed system
  • Monitor number of calls made to patients with the purpose of scheduling an appointment to ensure patients are not called more than six times within 60 days
  • Required to use telephonic software for all outreach calls made and answer and assist inbound calls received from patients who have been asked to call back
  • Required to follow approved PCC call script for 100% of outreach calls to patients
  • PCC will be assigned to an individual PSU provider to support their requests to schedule patients for follow-up appointments
  • PCC will direct and follow-up on all patient’s messages, concerns, and request for information making sure these calls are directed to appropriate level of staff to address issues. 100% of these calls will be returned to the patient within 48 hours
  • Required to meet established daily productivity goals
  • PCC will manage all medical record requests and send all PSU results to the PCP in a secured manner via email, fax or hand delivery
  • Obtain results from lab systems and upload into eCW patient documents, close order sets, and forward to the appropriate provider for review
  • Request specialty information at the request of providers to ensure accurate and timely information is available at time of PSU visit
  • Enter daily productivity of work complete by PSU team into the PSU utilization report and upload on weekly due date; work weekly error report on same day received
  • Establish and maintain effective and professional working relationships with patients, employees, vendors, and the public.
  • Performs all other related duties as assigned

Required Qualifications: 

  • High School Diploma / GED (or higher)
  • 2+ years of relevant work experience in a medical environment
  • 2+ years of experience with communication and customer service skills both in person and via phone
  • Intermediate level of proficiency with data entry 
  • Intermediate level knowledge of general office procedures
  • Intermediate level of proficiency in PC software skills such as Microsoft Word, Excel, Power Point, and other departmental specific programs/applications
  • Ability to properly drive and operate a company vehicle
  • Possess a valid driver’s license

Preferred Qualifications: 

  • ICD 10 and CPT coding experience
  • Prior experience with EMR computer applications
  • Working knowledge of medical terminology
  • Possess planning, organizing, conflict resolution, negotiating and interpersonal skills
  • Independent problem identification/resolution and decision-making skills
  • Must be able to prioritize, plan, and handle multiple tasks, demands simultaneously
  • Bilingual – English/Spanish

Physical & Mental qualifications:

  • Ability to lift, up to 25 pounds
  • Ability to push or pull heavy objects using up to 25 pounds of force
  • Ability to sit for extended periods of time
  • Ability to stand for extended periods of time
  • Ability to use fine motor skills to operate office equipment and/or machinery
  • Ability to receive and comprehend instructions verbally and/or in writing
  • Ability to use logical reasoning for simple and complex problem solving

 

The salary range for this role is $16.88 to $33.22 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
