Primary Responsibilities:

  • Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws
  • Oversee a team of on-site hospital social workers, as part of the larger Care Coordination Department 
  • Overall responsibility for the supervision and coordination of all workflow activities related to Social Work for the designated team
  • Adapts departmental plans and priorities to address business and operational challenges
  • Influences or provides input to forecasting and planning activities
  • Provides Social Work expertise to the team
  • Provides support with team’s UM/CM activities as necessary
  • Provides mentorship and training for team members
  • Build relationships with internal and external customers 
  • Assists in developing and implementing program initiatives, policies, and procedures 
  • Represents the department in assigned committees/meetings
  • Ensures state and federal compliance 
  • Facilitates regular team meetings to review complex cases, changes in procedures, etc.
  • Assists with all team member Human Resource activities
  • Supports excellent employee performance and encourages staff development
  • Works closely with upper management 
  • Prepares and submits activity or other reports as required

Required Qualifications:

  • Licensed Social Worker in WI (BSW, MSW, LSW, or LCSW)
  • 2+ years of experience in Case Management 
  • Electronic documentation experience
  • Knowledge of Medicaid and/or Medicare health care and insurance industry, including regulatory and compliance requirements
  • Proficiency in software applications that include, but are not limited to Microsoft Word, Excel, Power Point
  • Must live in the Waukesha WI Regional Area (this is an onsite position, not open to remote/telecommute work)
  • Must be able/willing to work onsite in a hospital environment daily

Preferred Qualifications:

  • Certification-Case management
  • Experience with EPIC  
  • Experience managing a team 
  • Experience in a hospital environment

  • Ability to assist with focusing activities toward a strategic direction as well as develop tactical plans, drive performance, and achieve targets
  • Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical Information in a manner that others can understand, as well as ability to understand and interpret complex information from others

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $89,900 to $160,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
