Position in this function provides support across the Care Transitions client portfolio through development and distribution of analytical presentations and strategic deep-dive reports that deliver insights to both Care Transitions operators and clients. Partners strategically with market leadership and cross-functional teams to inform business decisions with data and improve performance. This is a collaborative, individual-contributor role requiring healthcare experience, critical thinking skills, independence, leadership, strategic mindset, attention to detail, and an ability to multi-task in a fast-paced environment. This is a highly adaptable role that distills complex datasets into easy to understand visuals to accelerate and drive client experience and growth.

Primary Responsibilities:
- Partner with Market Leaders to develop and execute key internal/external strategic initiatives that drive incremental Operational Improvements to performance across clients and markets, including but not limited to: Key Performance Indicator Analysis, Consistent Client Reporting, Quality Objectives, Staffing, Partnership Opportunities, and other strategic Initiatives
- Support conversation in client-facing presentations, interfacing directly with external stakeholders
- Supports ad hoc analysis and special projects geared toward driving performance, scalability and problem-solving operational issues
- Assist in the development and training of Market Operations Analysts
- Serve as business lead designing, enhancing and automating reporting tools to deliver insights and actionable analysis to Market Leaders
- Develop relationships and lead collaboration efforts with key Client Performance stakeholders, including Analytics, Finance, Accounts Management, Clinical, Network, and Business Development teams to coordinate development of comprehensive client service offering
- Assist in the development and training/rollout of new reporting/dashboards
- Utilize Client Performance data resources to create actionable plans for achieving client performance targets
- Develop relationships with key client stakeholders to coordinate development of reporting and analytics offering
- Prepare internal performance reports for executive review
- Perform other duties and responsibilities as required, assigned, or requested
This is a complex, fast-paced, matrixed environment with a lot of moving parts. The challenge is in developing, enhancing and maintaining operational information and models. You’ll be expected to interpret and analyze large amounts of data from multiple sources and use it to tell a meaningful story.

Required Qualifications:
- 4+ years of experience working in cross-functional roles such as Clinical Operations, Consulting, Analytics, Quality Assurance, or comparable roles with health care industry experience
- Proven experience partnering with leadership in analyzing and interpreting large sets of data to tell a succinct, meaningful and actionable story using data visualization or other techniques
- Knowledge of value-based care models and Medicare Advantage health plan dynamics
- Intermediate to advanced level proficiency in MS Office Suite programs, particularly Excel and PowerPoint
- Proven ability to work on multiple projects simultaneously with minimal supervision while expecting and anticipating uncertainty and/or ambiguity in the work
Preferred Qualifications:
- Experience creating executive-level presentations and using data to tell a story
- Experience navigating and utilizing the clinical platform nH Coordinate
- Thorough understanding of IRF, LTAC, SNF, and HH criteria as well as the continuum of members through post-acute care
- Proven excellent written and oral communication skills and experience in presenting to varied audiences including senior level leadership

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The salary for this role will range from $71,200 to $127,200 annually based on full-time employment. We comply with all minimum wage laws as applicable.
