The Intake Case Aide is responsible for receiving, documenting, and managing referrals made to our Case Management Agency for folks seeking Long Term Care Services and Supports. The Intake Case Aide will utilize several different modalities including phone, email, and fax to receive and process referrals with timeliness and attention to detail. The Intake Case Aide will answer questions about how to access services and supports and provide information about available state programs, waivers, and providers related to an individual’s specific support needs. 

Schedule: Monday-Friday 8-5 MST

Primary Responsibilities:

  • Utilizing a professional and courteous demeanor, provide excellent customer service for customers and various internal departments and individuals in accordance with the mission, and values of the company
  •  Respect confidentiality and maintain confidences as described in the UHG Employee Handbook and acknowledged through signature by all employees. The ability to maintain confidentiality is a critical and essential component of this position
  • Serve as Intake Case Aide and provide administrative and case management support to Case Managers/HSS Coordinators as needed
  • Manage referrals for LTSS through multiple modalities
  • Collect appropriate and detailed information and documentation required for accessing LTSS
  • Provide information and options counselling for folks seeking services
  • Complete verbal consent lists and obtain verbal consent from members as needed
  • Interact with internal and external departments, agencies, advocates, medical providers, families, and individuals to connect members with what they need
  • Frequently handles conversations with individuals of varying emotional states. Responsible for diffusing the situation, providing information and empathy, and ensuring clients and their families are cared for as much as possible
  • Provide first point of contact coverage virtually in a cordial and professional manner. Determine their needs and provide information and/or notify the appropriate individual
  • Receive and route incoming calls as needed to ensure effective customer phone service. Respond to internal and external contacts in a patient, friendly and helpful manner
  • Maintain accurate and updated resource materials
  • In the event, at any time throughout the case management process, the case manager suspects an individual to be a victim of mistreatment, abuse, neglect, exploitation or a harmful act, the case manager shall immediately refer the individual to the protective services section of the county department of social services of the individual’s county of residence and/or the local law enforcement agency. Employees are obligated by statute, including but not limited to, Section 19-13-304, C.R.S., (Colorado Children’s Code), Section 18-6.5-108, C.R.S., (Colorado Criminal Code – Duty To Report A Crime), and Section 26-3.1-102, C.R.S., (Human Services Code – Protective Services), to report suspected abuse, mistreatment, neglect, or exploitation, are aware of the obligation and reporting procedures. 10. Other functions may be assigned and management retains the right to add or change the duties at any time
  • This position does not require travel
  • Include attendance at workgroups or regulatory meetings

Required Qualifications:

  • High School Diploma/GED (or higher)
  • 3+ years of experience in the field of LTSS (long term supports and services)
  • Must be able to work Monday – Friday, 8 – 5 pm MST

Preferred Qualifications:

  • Prior experience in a telephonic role
  • Intermediate level of experience using MS Office (Teams, Word, Excel)

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $17.74 to $31.63 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
