Discharge Case Manager – Remote working CST hours

Primary Responsibilities:
- The Discharge Care Manager RN is responsible for assessing barriers to a safe and efficient discharge process, identifying the relevant resources, and finally coordinating and driving an efficient discharge process with both internal and external stakeholders
- The DCM RN will prepare and attend CMO POD rounds to discuss case needs and prioritize patients/discharges/issues/facilities
- The DCM RN will initiate proactive outreaches to facility CM/UM/Discharge staff upon generating or receiving a case referral to manage the discharge, working collaboratively with the facility discharging staff to assist in resolving any discharge barriers and continuing to engage facility staff until the actual discharge
- Collaborate closely with internal and external partners to aggressively drive safe discharges to achieve higher daily discharge volume and overall lower LOS
- Continuously look for opportunities to improve and streamline DCM process, to maximize discharge efficacy & efficiency
- Cultivate and build relationships, especially where one does not exist or is challenging, with hospitals and their personnel to build a collaborative discharge management relationship
- Working closely with Families, parents, social workers and community services for infant’s care, safe DC planning and quality care
- Other tasks as requested by their manager

Required Qualifications:
- Bachelor of Science in Nursing
- Current, unrestricted RN license
- Certification neonatology or pediatric specialty
- 5+ years of recent experience in the Neonatal ICU inpatient acute setting
- 2+ years of experience of discharge planning
- Proficient in typing skills and software applications that includes, but is not limited to, Microsoft Word, Microsoft Excel, Microsoft PowerPoint and Microsoft Outlook
- Experience working with multiple health insurance products (Medicaid, Medicare, Commercial) within the insurance industry, including regulatory and compliance requirements
- Have a designated workspace and access to install secure high-speed internet via cable/DSL in your home
- Ability to work an 8 hour shift between M-F 7am-5pm central time
Preferred Qualifications:
- Case Management certification
- InterQual/MCG Guidelines or other nationally recognized practice guidelines
- Demonstrated ability to assist with focusing activities on a strategic direction as well as develop tactical plans, drive performance, and achieve targets
- Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action
- Proven excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from other

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you’ll find a far-reaching choice of benefits and incentives. The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
