The Health and Social Services Manager will lead a care management team while overseeing day-to-day clinical operations and health plan clinical initiatives to support achieving quality and affordability targets. The HSS Managers will support a team within different regions in Indiana. Responsibilities will include process improvement, execution of clinical decision processes, and team management, to include performance management and implementation of program initiatives.
If you reside in Central or Northern Indiana, you will have the flexibility to work remotely* as you take on some tough challenges.
Approximately 25-50% of field-based travel in Indiana.
Primary Responsibilities:
- Selects, manages, develops, mentors and supports staff in designated department or region
- Develops clear goals and objectives for performance management and effectively communicates expectations, and holds the team accountable for results
- In order to meet the unique needs of our members, have an intimate understanding of the contractual requirements
- Identify, select, structure, and prioritize process improvement projects, ultimately implementing changes to meet program requirements
- Conducts monthly audits of staff and participates in all other required audits and business monitoring
- Ensures standardized execution of workflow processes, including conducting performance audits, quality reviews, and compliance adherence
- Participates in training and coaching of direct reports as needed
- Collaborates across Optum and UHG and interacted with Medical Directors, Senior Leaders, Quality, and other stakeholders

Required Qualifications:
- Current, unrestricted independent licensure as a Registered Nurse or Master's in Social Worker in Indiana
- 3+ years of case management experience
- 2+ years of experience providing support or care coordination for older adults
- 2+ years of experience working within the community health setting in a health care role
- 2+ years of experience facilitating and working in coordination with an interdisciplinary team
- Intermediate level of proficiency in MS Office skills, including Word, Excel, Outlook, and PowerPoint
- Resident of state of Indiana
- Reliable transportation and the ability to travel within assigned territory to meet with employees, members, and providers
Preferred Qualifications:
- Experience with Long Term Service and Supports or Waivers Program
- Management experience in managed care setting
- Proven ability to navigate a TEAMs environment

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
