Schedule: 40-hour work week. Monday - Friday 8am - 5:00pm EST. (May be able to work from home as well as onsite the schedule will be determined by the hiring manager upon hire)

Primary Responsibilities:
- Contacts patients for the purpose of achieving established outreach program goals. This may include contacting patients via telephone, email and/or direct mailings to stress the clinical importance of receiving their recommended lab work and attending their medical appointments
- At the request of a Clinician, may place lab order and/or schedule appointments in electronic medical record system
- Generates reports using Healthy Planet Registry in Epic EMR to identify patients who require additional labs to be ordered as well as identifying patients who are over-due for labs
- Consults with clinicians to determine required population reports and/or patient lists for analysis and outreach
- Communicates and collaborates with care coordination staff to optimize patient care, and supports patient care plans by assisting with linkages to, coordination of, and monitoring identified service needs
- Recognizes and reports data inconsistencies, gaps in performance, and opportunities to improve results to appropriate personnel
- Review daily, weekly, and monthly HEDIS/STAR gap reports, registries, and dashboards to identify open care gaps
- Prioritize outreach based on measure impact, audit risk, and payer timelines
- Work measures across preventive care, chronic disease management, medication adherence, and wellness visits
- Conduct proactive outreach to members via phone, patient portal, and mail to close open HEDIS/STAR gaps
- Educate patients on the importance of preventive screenings, chronic condition follow‑ups, and medication adherence
- Address social, logistical, or access‑related barriers to care and escalate concerns when needed
- Deliver outreach using scripts and workflows aligned to payer and regulatory requirements
- Schedule appointments directly with primary care providers and specialists to close quality measure gaps
- Coordinate lab testing, imaging, and referrals required for measure compliance
- Ensure appointments are scheduled within payer‑required or measure‑specific timeframes
- Perform follow‑up outreach for missed or rescheduled appointments
- Ensure accurate and timely documentation in the EHR to support HEDIS abstraction and CMS STAR reporting
- Verify completion of services and confirm correct coding, diagnosis capture, and measure specific requirements
- Flag documentation issues or discrepancies for clinical reviews to prevent measure fallout
- Update care gap status and outreach outcomes in quality tracking tools
- Performs other duties as assigned

Required Qualifications:
- High School Diploma/GED
- 2+ years of experience in healthcare operations, quality improvement, population health, or care coordination. Direct experience working with HEDIS, STAR measures, or value‑based care programs
- 1+ years of direct experience working with HEDIS, STAR measures, or value‑based care programs
- 1+ years of computer proficiency experience to include Microsoft office (specifically excel worksheets)
- Ability to be remote and work independently
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
- Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
- Reside within commutable distance of 1 Dakota Drive, Lake Success, NY 11042
Preferred Qualifications:
- Proficient with data manipulation including the ability to develop spreadsheets, build data tables and extract data
- Knowledge of preventive care and chronic disease guidelines
- Proficiency with electronic medical record systems
- Knowledge and understanding of medical terminology
Soft Skills:
- Ability to work independently and as a team, and maintain good judgment and accountability
- Demonstrated ability to work well with health care providers
- Strong organizational and time management skills
- Ability to multi-task and prioritize tasks to meet all deadlines
- Ability to work well under pressure in a fast-paced environment
- Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $18 - $32 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
