As an RN with our Palliative Care team, you'll wear many hats and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Fewer hospitalizations, ER visits and costly service gaps; and a less stressed, more effective health care system for us all. Want more flexibility, want more autonomy? Primary Responsibilities:

  • Provide a complete continuum of quality care through close communication with patients via in-person or phone interaction
  • Support patients with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
  • Visit patients in their homes to discuss goals and available resources
  • Use a team approach to address the needs of patients and their families, including bereavement counseling, if indicated
  • Offer a support system to help the family help during patients' illness and in their own bereavement
  • Provide relief from pain and distressing symptoms
  • Other responsibilities as assigned

Required Qualifications:

  • Current, unrestricted NV RN license or temporary NV RN license with the ability to obtain permanent NV RN license upon expiration of temporary NV RN license
  • 2+ years of experience as a Registered Nurse (RN)
  • Bilingual (English/Spanish) communication skills
  • CPR certification or willing to obtain within 30 days of hire
  • Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area

Preferred Qualifications:

  • Hospice experience
  • Case management experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29 - $52 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
