The Community Outreach Coordinator is responsible for planning, implementing, coordinating, and evaluating community engagement, member education, and outreach initiatives that support UnitedHealthcare Community Plan of Kentucky's Medicaid program. This role serves as a key ambassador for the health plan by building and maintaining relationships with community-based organizations, health care providers, advocacy groups, public agencies, educational institutions, faith-based organizations, and other stakeholders throughout Kentucky.

The Community Outreach Coordinator engages members and community partners to increase awareness of health plan benefits, improve access to care, address social determinants of health, support health equity initiatives, and promote positive health outcomes. The position also supports compliance with Kentucky Medicaid contractual requirements related to outreach, member education, stakeholder engagement, community involvement, documentation, and reporting.

If you reside in Fayette County, KY or surrounding area, you will have the flexibility to telecommute* as you take on some tough challenges. This is a field-based position requiring approximately 75% of travel throughout Kentucky, including occasional evening and weekend work, to support community events, stakeholder meetings, educational presentations, and partnership development.

Primary Responsibilities:

  • Develop, cultivate, and maintain strategic partnerships with community-based organizations, advocacy groups, schools, health care providers, local health departments, family resource centers, youth service centers, chambers of commerce, faith-based organizations, shelters, and other community stakeholders
  • Represent UnitedHealthcare Community Plan of Kentucky at community events, health fairs, educational forums, stakeholder meetings, conferences, coalition activities, and Quality Member Access Committee activities
  • Identify opportunities to expand community partnerships that improve member access to health care services, education, resources, and support programs
  • Serve as a trusted liaison between community stakeholders and the health plan, gathering feedback and market intelligence to identify emerging needs, barriers to care, and opportunities to improve member experience and health outcomes
  • Plan and execute member and community outreach activities that increase understanding and use of Medicaid benefits, preventive care, care management, behavioral health resources, value-added benefits, population health initiatives, and available community supports
  • Conduct community presentations and educational sessions in virtual and in-person settings and promote awareness of programs such as WIC, SNAP, smoking cessation, parenting resources, behavioral health and substance use disorder services, and transportation assistance
  • Support health equity strategies and targeted outreach for underserved and high-risk populations, including individuals experiencing homelessness, former foster youth, victims of domestic violence, justice-involved youth, individuals with behavioral health needs, and individuals with chronic health conditions
  • Collaborate with Population Health, Care Management, Quality, and Community Engagement teams to address social determinants of health, reduce disparities, and implement community-based health improvement initiatives
  • Support outreach and continuity-of-care efforts for unhoused populations and justice-involved youth transitioning back into the community, including face-to-face connections to health care and community resources
  • Assist with the development, implementation, monitoring, and evaluation of the annual Community Outreach and Education Plan
  • Maintain accurate documentation of outreach activities, partnerships, event participation, stakeholder engagement, and program outcomes; track metrics and evaluate the effectiveness of engagement strategies
  • Prepare reports, presentations, and leadership or regulatory summaries, and ensure outreach activities and materials comply with Kentucky Medicaid contract requirements, CMS regulations, applicable marketing guidelines, and UnitedHealthcare policies and procedures

Required Qualifications

  • High school diploma (GED)
  • 1+ years of experience in community outreach, (ex: health care, public health, Medicaid, social services, nonprofit organizations, community relations, or a related field) 
  • 1+ year of experience developing and maintaining partnerships with community-based organizations 
  • Intermediate level of proficiency with Microsoft Office applications, including Outlook, Teams, Excel, and PowerPoint
  • Ability to travel extensively (up to 75%) throughout Kentucky, including occasional evenings and weekends
  • Reliable transportation for field-based community outreach activities
  • Must reside within Fayette County or surrounding areas

Preferred Qualifications

  • Experience working within Medicaid managed care or government-sponsored health care programs
  • Knowledge of Kentucky Medicaid benefits, services, regulations, and community resources
  • Experience working with diverse, underserved, or vulnerable populations
  • Knowledge of social determinants of health, health equity, and population health strategies
  • Certified Community Health Worker or similar certification
  • Bilingual language skills
  • Experience using Salesforce

  • Strong organizational, time-management, and project-coordination abilities, with the ability to balance multiple priorities

  • Ability to work effectively across Community Engagement, Population Health, Care Management, Quality, provider, and community partner teams
  • Demonstrated knowledge of regulatory compliance, member education, and applicable marketing requirements

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The hourly pay for this role will range from $29.00 to $52.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
