If you are located in Maryland Heights,MO, you will have the flexibility to telecommute* as you take on some tough challenges. 

Primary Responsibilities:

  • Manage a team of maternity members, including pregnant and postpartum individuals, with a focus on high‑risk pregnancies
  • Conduct comprehensive biopsychosocial and maternity‑specific assessments to identify medical, behavioral, and social needs; develop and manage individualized care plans
  • Provide education and support related to pregnancy, postpartum care, infant health, and maternal well‑being, including condition and medication education as appropriate
  • Address social determinants of health (e.g., housing instability, transportation, food insecurity, intimate partner violence, substance use) and connect members to appropriate community and state resources
  • Perform field‑based visits in homes, hospitals, prenatal clinics, or alternate settings to improve engagement, assess safety and support needs, and support care plan goals
  • Coordinate care and collaborate with OB providers, hospitals, behavioral health clinicians, community agencies, and internal care teams to ensure continuity of maternity care
  • Maintain frequent communication with members through phone, virtual, and in‑person interactions to promote engagement throughout pregnancy and the postpartum period
  • Support care transitions, including discharge from hospitals or residential facilities back to the home, ensuring follow‑up appointments and post-partum support are in place
  • Complete documentation timely and accurately to ensure program compliance and quality outcomes
  • Conduct field visits when members are difficult to engage telephonically or as directed by leadership to meet program needs

Required Qualifications:

  • Current LCSW (licensed clinical social worker) in the state of Missouri
  • 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care or case management
  • 2+ years of clinical experience in Maternal/Child Health 
  • Intermediate level of skills in computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
  • Access to high-speed internet from home
  • Dedicated workspace from home
  • Reside in MO
  • Access to reliable transportation and valid US driver's license

Preferred Qualifications:

  • Certification in Case Management (CCM)
  • Case Management experience
  • Home care / field-based case management
  • Medicaid, Medicare, Managed Care experience
  • Experience with vulnerable subpopulations include children and youth with special health care needs, adults with serious mental illness, domestic violence, children with serious emotional disturbances, members with substance use disorders and members with other complex or multiple chronic conditions

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29 - $52 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
