Position Highlights

You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The RN is a flexible team member who works alongside nurse practitioners (NPs) and physician assistants (PAs).  This is a field-based position, visiting members primarily in nursing homes.

Standard Hours:  Monday - Friday 8AM - 5PM Time; no nights, weekends, or holidays. 

Primary Responsibilities:

  • Collaborating with all members of the interdisciplinary care team around changes in condition and/or care coordination
  • Triage member needs to the appropriate care team member and level of care
  • Assist with transitions in care: outpatient and inpatient
  • Assist in assuring complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessment

Required Qualifications:

  • Associate degree or higher in Nursing (RN)
  • Active and unrestricted license in Texas
  • 1+ years of clinical experience 
  • Certified in Basic Life Support
  • Ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment
  • Access to reliable transportation to travel in between facilities


Preferred Qualifications:

  • BSN
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Saline Lock insertion experience 
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills including the ability to document medical information with written and electronic medical records


Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $60,200 to $107,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
