Position Highlights & Primary Responsibilities:

  • Independently manage a defined panel of primary care patients, providing longitudinal, relationship-based care

  • Perform comprehensive assessments, diagnoses, and treatment of patients in an ambulatory primary care setting

  • Manage acute and chronic conditions and serve as the author and supervisor of patient care plans

  • Make clinically appropriate determinations regarding in office management versus referral to specialty or emergency care

  • Manage inbox responsibilities, including test results, patient messaging, medication management, and referrals

  • Coordinate care with interdisciplinary team members including RNs, pharmacists, population health, behavioral health, and specialists

  • Partner with patients and families to support shared decision making and whole person care

  • Provide consultation regarding hospitalized patients within the assigned panel

  • Support practice efficiency, quality initiatives, and standardized clinical workflows

  • Participate in a shared, site‑based call structure aligned with team‑based primary care

  • Spend less time charting with DAX AI ambient scribe technology

  • Practice efficiently with SmartRx and robust EPIC tools, including 30+ medication templates

Required Qualifications:

  • Board Certified as a Nurse Practitioner (NP) or Physician Assistant (PA)

  • Must have at least 3 years of paneled APC experience

  • Unlimited Licensure in the state of Massachusetts

  • Active DEA License

Preferred Qualifications:

  • Experience working within a Team Care model

  • Fluency in EMR systems & technology

  • EPIC experience a plus

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
