The Field Based Community Health Worker (CHW) position will actively engage health plan members and clinical contacts to increase their engagement with and increase their utilization of their health plan benefits and offerings. The CHW is expected to serve as a field-based resource, meeting with members in direct outreach in their homes and communities and representing the organization in a positive and professional manner.

Working Schedule: This position is full time, 40-hour work week, Monday - Friday between the hours of 8:00am - 5:00 pm Central Standard time. Occasionally may work outside these hours due to member's needs. The schedule will be determined by the manager upon hire.  This position is a field-based position with a home-based office. You will work from home when not in the field.`

Location:Milwaukee, and surrounding communities in Wisconsin(Ability to rent a car if exceeding 100 miles per day)

If you reside within the state of Milwaukee, WI, you will enjoy the flexibility to work in both the field as well as remotely* as you take on some tough challenges.

Primary Responsibilities:

  • Engage members at their residential homes, senior living apartments, long term care facilities, or within assisted living communities

  • Provide member education regarding programs and their benefit plan offerings

  • Coordinate and schedule members for appropriate clinical service visits, ensuring timely access to care.

  • Build member trust and foster their relationship with Optum health care providers

  • Actively engage with the member to remove any obstacles hindering or preventing them from utilizing their health plan benefits

  • Proactively address any challenges the member may encounter in the future to improve their experience and increase their utilization of benefits and provider engagement, escalate concerns and barriers as appropriate in support of the member

  • Attempt to complete members' appropriate assessments, identify other health plan needs and provide community Resources

  • May complete the Bone density screening-Osteoporosis Management in Women who had a fracture (OMW) for members as appropriate

  • Utilize internal electronic medical record system to document member engagement attempts and ensure improved coding accuracy

  • Plan routes for best time management of schedule and travel time/distance to optimize engagement of identified members

  • Utilize scheduling tools to schedule members for various types of visits; ultimately resulting in an increased Stars gap closure and reducing medical expenses

  • Performs other duties as assigned

Required Qualifications:

  • High School Diploma/GED
  • 3+ years of experience working within a community health setting or community outreach experience
  • 2+ years of professional experience working with diverse multi-ethnic and socioeconomic populations
  • 1+ years of field-based experience 
  • Intermediate level of computer experience with the ability to navigate a Windows Environment, especially Microsoft Excel
  • Ability to keep all company sensitive documents secure  
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service 
  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy
  • Active and unrestricted driver's license and access to reliable transportation enabling you to travel to member homes and communities within a designated area 
  • Must reside within a commutable distance to Milwaukee, and surrounding communities in Wisconsin
  • Must be able to plan routes for best time management of schedule and travel time/distance 
  • Ability to have flexibility with scheduled hours based on member's availability and/or needs

Preferred Qualifications:

  • 3+ years of experience working with vulnerable populations
  • Experience working in sales / marketing
  • Licensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, or Medical Assistant
  • Bilingual fluency in Spanish and English
  • CHW Certification
  • Experience working in a managed care organization
  • Knowledge of Medicare/Medicaid population
  •  Telecommuter/work from home experience

  • Able to exhibit good judgement for improved patient outcomes with the ability toconsult leadership and escalate as appropriate

  • Ability to work independently and maintain good judgment and accountability

  • Demonstrated ability to work well with health care providers

  • Strong organizational and time management skills

  • Ability to multi-task and prioritize tasks to meet all deadlines

  • Ability to work well under pressure in a fast-paced environment

  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.00 to $36.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
