This position follows a hybrid schedule with three in-office days per week.

Primary Responsibilities: 

  • Assist in the care of patients through telephonic outreach to identify care needs and barriers to care, connect patients to community resources, and schedule hospital follow up appointments and AWV's as needed
  • Obtain pertinent medical records for discharged patients to support provider decision making during HFU appointments
  • Complete medication review of new and changed medications for patients discharged from a facility
  • Support and initiate referrals and tracking of patients to internal and external patient care support programs 
  • Coordinate with post-acute services to ensure appropriate patient support has been initiated
  • Escalate concerns and urgent items to providers for timely resolution to support optimal health outcomes
  • Submit charges/create billing encounters for patient outreach to support gap closure
  • Utilize tools and reports within the EMR to identify high risk patients and support connecting those patients to appropriate care to minimize hospitalizations and poor outcomes
  • Document patient interactions accurately in appropriate systems/logs and ensure compliance with relevant  procedures and regulations which support provider documentation of TCM metrics
  • Demonstrate understanding of relevant computer systems and software applications, and ensure the working condition and maintenance of necessary equipment
  • Work as part of a team and collaborate effectively with providers, colleagues and management
  • Positions in this function assist in the care of the patient by following through on Provider requests for coordination of care related to hospital discharge
  • Individuals in this function typically have certification or are working towards their certification as a Medical Assistant 
  • Works on simple tasks using established procedures
  • Ability to work as part of a team and collaborate effectively with others - asking for guidance when necessary

Required Qualifications: 

  • High school graduate or GED
  • Completion of a Medical Assistant program, or Military Medic program 
  • Current CPR and / or BLS certification or ability to obtain certification within 30 days of hire
  • Competence in using relevant computer systems and software applications (e.g., MS Office Suite & Epic)
  • Proficiency in medical terminology and patient safety procedures
  • Proven solid communication skills to translate and communicate medical information clearly to patients and colleagues

Preferred Qualifications:

  • Certification as a Medical Assistant or actively working towards it
  • 5+ years of experience in a healthcare setting - preferably as a Medical Assistant or Care Coordinator
  • Experience with EPIC EMR
  • Understanding of relevant state and federal regulations (e.g., HIPAA) and scope of practice
  • Familiarity with various insurance programs/products (e.g., Medicare, Medicaid)
  • Bilingual - Spanish

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits re subject to eligibility requirements). The hourly pay for this role will range from $16.00 to $29.00 per hour ($19.00 to $25.00 for candidates in Scottsdale, AZ) based on full-time employment. We comply with all minimum wage laws as applicable.
