Schedule: Monday to Friday from 8:00 am to 5:00 pm CST

Location: Position is remote; if live within 50 miles of a corporate office, switch to hybrid model

You'll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges.

Primary Responsibilities:

  • Answers phones and schedules screening appointments for patients and providers using the scheduling system.
  • Supports outbound patient follow‑up calls, including return calls, appointment reminders, and rescheduling as needed.
  • Schedules patient visits in accordance with established access‑to‑care standards, including post‑discharge appointment timeframes.
  • Maintains accuracy of scheduled appointments, ensuring correct visit type, facility type, and patient eligibility are reflected in the system.
  • Assists with day‑to‑day maintenance of provider schedules, including administrative blocks, PTO, meetings, and training sessions.
  • Coordinates rescheduling efforts related to provider availability changes, cancellations, or operational needs.
  • Follows up on patient no‑shows and missed appointments to support continuity of care.
  • Monitors shared team inboxes and task queues, routing messages, requests, and documentation to the appropriate internal teams according to established workflows.
  • Processes referrals and confirms receipt to ensure documentation is complete, accurate, and appropriately routed.
  • Requests and manages medical records electronically in compliance with organizational policies and regulatory requirements.
  • Conducts patient outreach in accordance with established protocols for patients who are difficult to contact or unable to be reached.
  • Documents work clearly and accurately in the EMR and other systems in accordance with Care Coach Connect policies and procedures.
  • Demonstrates professionalism, courtesy, and sensitivity when interacting with patients, caregivers, providers, and staff in a virtual environment.
  • Adheres to written protocols and procedures and supports quality, efficiency, and patient satisfaction initiatives.
  • Performs other related duties assigned, consistent with the scope of the role.

Required Qualifications:

  • High school graduate or GED
  • 1+ years of related work experience including data entry
  • 1+ years experience in navigating Internet tools such as Google Maps and Map Quest
  • 1+ years working knowledge of medical office procedures and medical terminology.
  • 1+ years and customer service skills required
  • Basic level of understanding with Microsoft Office applications to include Excel, Word, and Outlook

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will rangefrom $18 to $32 annually based on full-time employment. We comply with all minimum wage laws as applicable.
