As a Telephonic Maternal Health Registered Nurse Case Manager who is Bilingual Spanish/English, you will work with our Medicaid members primarily focusing on our Spanish speaking population, wearing many hats. you will act in a liaison role with Medicaid members who are pregnant or were recently pregnant to ensure appropriate care is accessed as well as to provide home and social assessments and member and family education. The Maternal Health RN Case Manager will coordinate care that focuses on supporting members' medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improve quality of care. Interest in learning to work with maternity population is important for this role. You'll need to be flexible, adaptable and, above all, patient in all types of situations. You will have the ability to work remotely as you take on some tough challenges.

Primary Responsibilities:
- Engage members telephonically to complete comprehensive, ongoing needs assessments to identify medical, behavioral, functional, cultural, and socioeconomic needs
- Develop and implement person-centered care plans to address needs including management of chronic health conditions, health promotion and wellness, social determinants of health, medication management and member safety in alignment with evidence-based guidelines
- Partner and collaborate with internal care team, providers, and community resources/partners to implement care plan
- Provide education and coaching to support member self-management of care needs and lifestyle changes to promote health
- Support proactive discharge planning and manage/coordinate care transition
- Advocate for members and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team

Required Qualifications:
- Current, unrestricted WA state or multi-state RN licensure
- 2+ years of Maternal clinical experience
- 1+ years of experience with MS Office, including Word, Excel, Outlook, and TEAMs
- Intermediate level of computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
- Designated, quiet workspace and access to install secure high-speed internet via cable/DSL in the home
- Bilingual proficiency in English and Spanish (verbal and written)
- Solid telephonic engagement and motivational interviewing skills
- Adherence to a Pacific Standard Time (PST) work schedule
Preferred Qualifications:
- CCM certification
- 1+ years of Case Management experience coordinating care for individuals with complex needs
- Experience working in team-based care
- Experience with NICU and/or Pediatric populations
- Experience with vulnerable subpopulations include pregnant/post-partum women with special health care needs, members with substance use disorders and members with other complex or multiple chronic conditions
- Proven background in Managed Care
- Reside in the state of Washington
- Reside in the West Coast Region of the U.S. (WA, AZ, NM)
Physical Requirement:
- Ability to remain stationary for long periods of time to complete computer or tablet work duties

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.94 to $54.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
