The Clinic Coordinator is responsible for the completion of processes and protocols that support efficient and effective patient care. As a member of the central clinic operations support team, they provide support to designated region(s). Contribute to day-to-day patient care through central care coordination, patient outreach, specialty care coordination, and system navigation. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service, and adhere to Lean processes. Supports the teams in meeting financial, clinical, and service goals. Coordinators will work from a regional administrative office and may occasionally travel to a clinic within the region.
Primary Responsibilities:
- Engage in activities to ensure all needed patient information is available in the EMR prior to the patient's visit, including:
- Complete chart review and necessary preparations of patient record for designated care team prior to visit
- Review patient's chart for various items, including:
- Last visit note for additional needs or follow-ups, including consult and surgical notes, imaging, and lab results that are not documented
- Open quality care gaps and HEDIS measures
- Recent hospital discharge
- Initiate and conduct calls to patients prior to appointment date to prepare patients for visit, including:
- Confirm appointment, reason for the visit, review prescription refill requests, and determine if they have new medical records since last visit
- Complete visit preparation activities ahead of patient visit, including:
- Reconcile data across operational systems and add supplemental data
- Generate standing orders in EMR as applicable
- Conduct follow up on referrals associated with open care gaps, hospital discharges or other open items ahead of the patient visit
- Send tasks for completion to other departments, request medical records when needed from hospitals, ERs, UCs, labs and other health services providers
- Document medication refill requests, if applicable
- Document changes in EMR
- Manage pursuit lists and conduct outreach to patients as needed
Other:
- Participate in planning, coordinating, and scheduling care in conjunction with the multidisciplinary team in accordance with the Plan of Care
- Facilitate and / or participate care team meetings
- Review status of appointment with Community Volunteer Coordinator and schedule appointments as needed
- Perform other duties and assignments as required

Required Qualifications:
- High School Diploma/GED (or higher)
- Medical Assisting Certification
- 1+ years of health care or customer service work experience with solid communication skills
- Experience with EMR computer applications
- Working knowledge of general office procedures
- Working knowledge of the healthcare industry, roles, and terminology
- Basic level proficiency in Microsoft Office
Preferred Qualifications:
- Experience with medical insurance
- Bilingual
- Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others
- Ability to work in a fast-paced environment
- Work collaboratively with various groups to achieve specific team goals
- Ability to work independently and as a team and maintain good judgment and accountability
- Solid organizational and time management skills
- Ability to multi-task and prioritize tasks to meet all deadlines
Physical Requirements:
- Interaction with others that requires employee to verbally communicate, as well as hear and understand spoken information
- Operate computers, telephone, office equipment, including manipulating paper requiring the ability to move fingers and hands
- See and read computer monitors and documents
- Ability to travel to multiple work locations
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $18.00 to $32.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
