Care at Home is a collaborative, population health-focused care delivery model serving Dual Eligible Special Needs Plan (DSNP) members-individuals with significant medical, behavioral, and social complexity, including serious mental illness (SMI) and substance use disorders (SUD). In this role, you will partner with an interdisciplinary care team to deliver high-quality, evidence-based behavioral health care.
As a Behavioral Health APC (PMHNP), you will play a critical role within an interdisciplinary care team-bringing psychiatric expertise into a home-based and population health model of care. You will support both direct patient care and consultative care.
If you have Oklahoma licensure, you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:
- Conduct telehealth-based psychiatric evaluations for patients with serious mental illness and substance use conditions
- Diagnose, treat, and prescribe evidence-based pharmacologic and non-pharmacologic interventions
- Provide bridge stabilization through medication management and follow-up care for high-risk patients
- Work with interdisciplinary care team to ensure connection to community mental health services.
- Act as a behavioral health consultant to an interdisciplinary care team including providers, RNs, and social workers
- Support management of depression, anxiety, SMI, and SUD
- Provide real-time and asynchronous consultation, including treatment recommendations and care planning
- Help triage patients to appropriate levels of care, including specialty mental health and community-based resources
- Support care continuity by partnering with external behavioral health providers and systems of care
- Facilitate engagement in specialty care when needed
- Deliver behavioral health education to interdisciplinary teams managing medically complex patients
- Participate in case conferences, care rounds, and team-based decision-making
- Contribute to a culture of continuous quality improvement and clinical excellence
- Document all patient encounters, consults, and care coordination efforts in the EMR
- Communicate clearly and effectively across care teams and community partners

Required Qualifications:
- Board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Active, unrestricted NP license with prescriptive authority
- Experience managing complex psychiatric conditions and/or SUD and SMI
- Willingness to obtain multi-state licensure
- Solid ability to collaborate within interdisciplinary teams
- Comfort delivering care via telehealth and technology-enabled platforms
- Ability to synthesize complex medical and behavioral health data into actionable care plans
- Excellent communication, clinical judgment, and a solution-oriented mindset
- Driver's license and access to reliable transportation
Preferred Qualifications:
- 3+ years of post-certification clinical experience
- Experience in consultation-liaison psychiatry, population health, or integrated care models
- Experience with geriatric, DSNP, or medically complex populations
- Telepsychiatry experience

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
