As the Home and Community Bases Services Case Coordinator, you will oversee the services provided to a caseload of clients on a daily basis. This responsibility includes interfacing with clients/families/employees to ensure quality service delivery. You will also be responsible for day-to-day staffing of cases, problem solving, assisting in the hiring process, providing orientation to new caregivers, payroll/billing, and ensuring customer satisfaction. As the Case Coordinator must be able to function with minimal onsite supervision and may perform some or all of the duties listed below.


Primary Responsibilities: 

  • Coordinate daily client case scheduling, ensuring appropriate caregiver coverage and ongoing communication with clients and families
  • Manage staffing operations, including anticipating workforce needs, supporting hiring and orientation, and participating in on call rotation
  • Oversee payroll, billing, and data accuracy by coordinating, entering, and auditing weekly payroll/billing and maintaining client and employee records
  • Conduct and support client consultations, on site home and supervisory visits, and monitor customer satisfaction, escalating issues as needed
  • Ensure compliance with company policies, state and funded program requirements, pay rate guidelines, overtime approvals, and incident reporting

Required Qualifications: 

  • High school education or equivalent
  • Demonstrated ability to coordinate many activities at one time
  • Demonstrated strong customer service skills

Preferred Qualification: 

  • Scheduling and data entry experience

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $17.98 to $32.12 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
