This position will be on – site at assigned skilled facilities. The Skilled Facility Navigator CAC Position is responsible for assisting our Optum NV Medicare Advantage members at the facility bedside upon admission to the skilled facility up through discharge focusing on ensuring a safe discharge plan and preventing a return to acute facility admission for our higher risk population.  

This position is full-time, Monday – Friday. Employees are required to work our normal business hours of 8:00 AM – 5:00 PM PST. Our office is located at 2716 N Tenaya Way, Las Vegas, NV.

We offer weeks of paid on-the-job training. The hours during training will be 8:00 AM – 5:00 PM PST from Monday – Friday.

Primary Responsibilities: 

  • Will collaborate and work closely with Optum SNFists and facility personnel.
  • Manage intake of SNF members
  • Work with SNF facilities and the clinical team to manage requests for services from members and / or providers
  • Process incoming and outgoing referrals, and prior authorizations, including intake, notification and census roles
  • Document and assist the clinical staff with setting up documents / triage cases for Clinical Coverage Review 
  • Handle resolution / inquiries from members and / or providers

Required Qualifications:

  • High School Diploma / GED OR equivalent years of experience
  • 1+ years of customer service experience
  • Experience with Windows – based programs, including Microsoft Word, Microsoft Excel, and Microsoft Outlook
  • Ability to work any shift between the hours of 8:00 AM – 5:00 PM PST from Monday – Friday
  • Must be 18 years of age OR older

Preferred Qualifications:

  • 2+ years of clinical customer service experience
  • Experience within the health care Industry working in a hospital, physician’s office, OR medical clinic setting and with health care insurance
  • Previous clerical OR administrative support experience

Nevada Residents Only: The hourly range for Nevada residents is $16.54 – $32.55 per hour. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
